HOW TO CALL IN SICK
1. すみませんが、きょうは。。。がいたいですので、休ませてもらいませんか？
Excuse me, today my… hurts; could I take the day off?

2. すみませんが、きょうは。。。がいたいですので、お休みをいただけませんか？
Excuse me, today my… hurts; could I take the day off?

3. 社長、すみませんが、不良体調ですので、お休みをいただけませんか？
I'm sorry, director, but I'm not feeling well today; could I take the day off?
（不良体調：ふりょうたいちょう: poor state of health）

The next day, when you are better and come back to work, your director or your colleagues may ask how you are; you can say:

昨日は迷惑をかけてしまいました、すみません。元気になりました、よろしくおねがいします。
I'm very sorry for the trouble I caused yesterday. I'm feeling well again now, and I look forward to your continued support
